The incidence rates in countries like Denmark and Sweden highlight the urgent need for effective prevention and treatment strategies.
One might ask, if HPV vaccination were widely adopted for both genders, could we see a decline in HPV-positive oropharyngeal cancer cases in the future? Some countries, including the UK, Sweden, and Denmark, have already implemented public vaccination programs, but the long-term impact on cancer incidence remains to be seen due to the time lag between vaccination and potential cancer development.
Immunotherapy and Biomarkers: A New Frontier
The biological distinction between HPV-positive and tobacco/alcohol-induced oropharyngeal cancer presents an intriguing opportunity for targeted treatment. HPV viral antigens can serve as immune system targets, making immunotherapy a promising avenue.
The approval of anti-PD1 immune checkpoint inhibitors in 2017 marked a significant shift. These antibodies enhance antitumour immunity by blocking the PD1 checkpoint. While initial expectations were high for their efficacy in HPV-positive disease, studies like CheckMate 141 showed similar survival outcomes across HPV subgroups.
Despite this, anti-PD1 therapy has improved outcomes in specific populations, solidifying its role as the preferred clinical platform for newer strategies. The intellectual property (IP) landscape surrounding these inhibitors, including patents and SPCs, ensures their market dominance well into the future.
